There are two important steps we need from every player in order to step on the ice for the first time. It is VERY important that these are completed. 

Complete the  COVID acknowledgement here: https://forms.gle/Arrzw8gwVbfgDNcL6

Print and complete the waiver for the Le Sueur Community Center.  The community center is requiring this to be a physical paper copy turned in as your player(s) enter the rink for the first time
